the most common cause of impa is bacterial arthritis. this most frequently involves staphylococci (such as staphylococcus intermedius) and streptococci (such as streptococcus sanguis). a variety of opportunistic organisms, such as enterococci, pseudomonas aeruginosa, and enterobacteriaceae, may also be found in the joint. after determining the number of cells in the synovial fluid, the laboratory also should examine the synovial fluid for the presence of leukocytes and compare the cells with those seen in a leukocyte-rich synovial fluid.


in addition to bacteremia, other potential causes of staphylococcal impa include trauma, inadequate antibiotic therapy, and a foreign body reaction (such as a retained catheter tip or another foreign body). staphylococci are also frequently isolated from cases of atraumatic joint disease that are associated with a history of trauma.


in some cases, cats without a detectable cause for impa have what is known as spontaneous impa. this can occur with an unknown cause (such as trauma or a disease such as systemic lupus erythematosus) or with a known cause (such as a specific bacterial infection). spontaneous impa can be an initial sign of a disease such as idiopathic granulomatous polyarthritis, which is a common disease of older cats that affects primarily the hips. this disease can be distinguished from impa because it causes a more generalized lameness, which is characterized by the tendency to avoid weight bearing on the affected limb, than that associated with impa.
